As a California State Licensed in-home, non-medical caregiving agency, we understand that every client’s needs are unique, and we are committed to providing the highest level of service from the very first interaction.
The onboarding process begins with an initial client inquiry, where we speak with the client, a family member, or someone in a supportive role. During this initial phone conversation, we gather essential information about the client’s current situation and care requirements. This conversation also serves as an opportunity to determine our ability to meet the client’s expectations and ensure we can provide the appropriate level of care. Our goal is to answer questions, explain our services, and ensure both the client and their family are comfortable moving forward.
Once the client expresses interest in the services we provide, we schedule an in-home assessment to gain a deeper understanding of client needs and preferences. During this visit, we consult with the client and any family members or decision-makers to review the care needs in detail. This process allows us to evaluate the client’s daily living requirements, specific care preferences, and lifestyle. We also conduct a home safety review, identifying any potential risks and making recommendations to enhance safety and comfort in the home. For those who may qualify we will help identify any programs, community resources, or benefits that might be available for our clients such as Long Term Care Insurance, VA benefits, etc. This comprehensive assessment enables us to create a tailored care plan that reflects the client’s unique situation.
In addition to providing non-medical home care services we will also assist in coordinating and referring other services such as durable medical equipment, home modifications, emergency response systems, community based programs, transportation services and coordinating medical appointments.
Once we gather all pertinent information, Assisting Hands® Dana Point works with the client and their family to develop a customized care plan. This plan addresses all care requirements, preferences, and safety concerns, ensuring that the client receives the personalized attention they deserve. Once the care plan is finalized, we begin the process of matching the client with a suitable caregiver. Depending on the frequency and duration of the caregiving services, we may assign a single caregiver or a team of caregivers to fill shift assignments, ensuring continuity and quality of care.
At Assisting Hands® Dana Point, our commitment to quality care doesn’t end once services begin. We maintain ongoing communication with the client and their loved ones to provide feedback, adjust the care plan as needed, and ensure important information is shared. Additionally, we conduct both supervisory and reevaluation visits to monitor the client’s status, caregiver performance, and fine-tune the care plan as necessary. This continuous oversight ensures that we are consistently meeting our clients’ evolving needs and maintaining the highest standards of care.
