Once the client expresses interest in the services we provide, we schedule an in-home assessment to gain a deeper understanding of client needs and preferences. During this visit, we consult with the client and any family members or decision-makers to review the care needs in detail. This process allows us to evaluate the client’s daily living requirements, specific care preferences, and lifestyle. We also conduct a home safety review, identifying any potential risks and making recommendations to enhance safety and comfort in the home. For those who may qualify we will help identify any programs, community resources, or benefits that might be available for our clients such as Long Term Care Insurance, VA benefits, etc. This comprehensive assessment enables us to create a tailored care plan that reflects the client’s unique situation.
In addition to providing non-medical home care services we will also assist in coordinating and referring other services such as durable medical equipment, home modifications, emergency response systems, community based programs, transportation services and coordinating medical appointments.
Once we gather all pertinent information, Assisting Hands® Dana Point works with the client and their family to develop a customized care plan. This plan addresses all care requirements, preferences, and safety concerns, ensuring that the client receives the personalized attention they deserve. Once the care plan is finalized, we begin the process of matching the client with a suitable caregiver. Depending on the frequency and duration of the caregiving services, we may assign a single caregiver or a team of caregivers to fill shift assignments, ensuring continuity and quality of care.
